What to bring to your first appointment
Thank you for choosing Neurodevelopmental Therapy Services. To begin the new client process, please fill out the following forms listed below. You may fill them out ONLINE and submit to us by pressing the submit button when you have completed the forms. Another option is to PRINT the forms, fill in and fax within 24 hours to the appropriate clinic where the initial evaluation will take place. Please be sure each form is complete, signed and dated before you arrive so that your appointment may begin on time.
What to bring to your first appointment:
- Drivers License
- Insurance Card
- Client Information Form
- Insurance Information Form
- Prescription from physician for OT/PT/ST/MT or Aquatics
To be completed at the clinic prior to appointment:
- Client Rights and Responsibilities Form
- NTS Financial Guidelines Form
- HIPPA Acknowledgement Form
- HIPPA Guidelines
- NTS Photo Authorization Release
We ask that if you submit your paperwork ONLINE, to do so within 24 hours of your scheduled appointment. We also advise you to arrive 15 minutes early in order to complete the other required documentation.
